I consent to my/my son's image being used for Boys' Brigade publicity purposes.

By ticking here, I authorise the leader\s in charge to arrange for my child/myself to receive such first aid, medical or surgical treatment as the leader may deem necessary at any time during any of the activities of The Boys’ Brigade Victoria. I further authorise the use of Ambulance and or anaesthetic by a qualified medical practitioner if in his / her judgement it is necessary. I accept all responsibility for payment of all expenses associated with such treatment. I agree to indemnify and hold harmless The Boys’ Brigade Victoria, and any of the individual staff or voluntary leaders against all claims, demands, suits and liability of whatever nature and howsoever arising out of the injury to the child or myself and the relevant activity being undertaken.
